Care Planning Workshop
Develop a child focused and holistic approach to care planning and be aware of the key factors and details required in each section of the care plan and how this links with other planning documents and tools.
Description
The first part of the day will be and information, and resource sharing opportunity providing guidance and tools for practice including a “Care Planning Pack”. The session will be concluded with a group exercise looking at a case study and formulating a single assessment.
The second part of the day will enable you to then use this assessment to develop a care plan.
The final part of the day will be a session of reflective practice looking at how this informs our practice and what we see as a gold standard care plan.
The workshop will include power point presentations, break out group working, some written exercises and collective and reflective discussions with the support of two trainers.
By the end of the Workshop you will :
Be clear about the legislative framework and expectations of care planning
understand the importance of partnership with families and carers in care planning
Have developed a child focused and holistic approach to care planning.
Be aware of the key factors and detail required in each section of the care plan and how this links with other planning documents and tools
Be able to differentiate between information, analysis, and actions
Be able to set SMART short and long terms aims and objective
